Parental Consent Form

With the completion of this form, I do certify that I am the Parent or Legal Guardian of the child/children listed below and do give my permission for my child/children to participate in the ministries of the Lava Avenue Baptist Church including all the events, games, activities, camps, rallies, teachings, lessons, music, and such like.  I give my permission to LABC to transport, visit, contact, teach, provide spiritual counsel and mentoring, help my child become saved and maintain control through rules and reasonable hands off disciplinary actions (such as separating a child from others, calling parents, taking away of a privilege and etc.)  I do not hold LABC responsible for injury or harm in the course of any church activities or by other people who are participating in those activities.  I understand that this is an Independent Baptist Church and my children will be spiritually taught accordingly.
